假期又要來寫長文小說了，不過今天暫且放下去年選舉前「萊爾」、 「酸奶」、「臭奶」的情緒，好好聊一下醫師生涯的甘苦談…
30多年醫師生涯， 現今自認醫術純熟了，但回想過去，其實也有經歷菜鳥的階段，記得20多年前有次去幼兒園幫幼童體檢，看到一個瞳孔放大的小男孩，當時心頭一驚，以我們神經科醫師的立場， 瞳孔放大， 那豈不快掛了， 但看那小男孩活力甚佳， 滿園飛舞， 心理實在困惑， 後來詢問老師才知道家長每晚都幫他點散瞳劑來預防近視加深， 當時覺得真的糗斃了， 這麼小小的tip都沒想到，菜鳥就是菜鳥。
大家不要覺得好笑，實戰經驗和腦袋知識有時實在搭不起來 ，記得我已經披醫師長袍了，有次被別的醫師照會去看一對姊弟，他們的病史是下肢突然僵直、動彈不得，當時在病房左敲敲右看看，實在找不出神經病杜在哪個level ，只好回頭再仔細詢問病史， 後來媽媽才透漏因小孩學游泳， 自己對他們要求甚高，於是昨天游泳課前讓他們喝了肌酸飲料，沒想到今天上課回來就變這樣，後來住院醫師去review 了paper ，說可能是這種飲料的某個成分造成(但我沒經驗)，還有另一可能是媽媽要求過高，姊弟的心理壓力造成身體病症，這和hysteria純粹因心理壓力裝出來的又不同， 不過還好姊弟倆打了點滴兩三天，代謝物排出後， 症狀就改善了，這病例讓我印象最深刻的不是這種飲料的神奇效果，而是住院時弟弟因兩眼瞳孔放大， 被年輕醫生逮去做腦部MRI，典型的沒把神經病灶level搞清楚就去做昂貴檢查的案例 ，不過學生被我K幾次後就記得了， 沒抽搐，意識滿分，無12對腦神經病變，那麼該懷疑的神經病杜還是往腦部以下找吧， 後來這對姊弟證實是神經unit 最終端的肌病變，至於有沒有心理因素貢獻，不得而知。
醫院是小型社會的縮影， 有病須靠醫學常識診斷， 有其他隱情要靠敏銳度細細探究…
『我剛當上主治醫師不久，遇到一位單親媽媽帶著三個姊弟來門診就診，三姊弟的主訴是晚上夜咳得很厲害，咳到睡不著 ，我給的診斷是過敏性氣管炎，打算開些支氣管擴張劑、抗組織胺和短期類固醇給他們，但媽媽一直強調晚上會出現極大的喘鳴聲，合併呼吸困難，怕他們缺氧，要求要住院治療，當時聽診實在不覺得三姊弟會有如媽媽描述的嚴重症狀，但有些過敏病人，的確只遇到夜間冷空氣時才發作，媽媽一直強調症狀嚴重， 我只好簽床讓他們住院。
第二天一大早去查房， 護理師一直跟我抱怨說三姊弟大夜班在病房內嬉鬧，跑來跑去影響其他病人的休息，且他們晚上根本沒咳嗽，這時媽媽聽了趕緊接口，說她會約束小孩，再三強調他們半夜咳很厲害，要求我繼續讓他們住院。或許吧，小孩第一天入院，凡事新鮮，不受控制也很正常，於是同意他們再留一天。
隔天再去查房，護理師反映媽媽雖有盡力約束小孩，但小孩仍在自己的病房內嬉鬧，他們住的是三等床(當時本院尚未整建，還有820這種住5個人的病房)，會影響同病房的其他病人，聽了主護護士的描述，媽媽有些心急，霹靂啪啦、滔滔不絕地解釋一堆理由，但我仔細聽診、評估，覺得三姊弟就算有症狀，也沒嚴重到需要住院的程度， 當下立刻要求他們出院返家，回門診追蹤即可， 接下來這一幕著實讓我震驚…
媽媽把我叫到一個四下無人之處，坦承她欺騙了我，她要求住院的目的，只為求小孩能得到每日3000元的住院保險給付，她還強調她有腦瘤，接下來就要去台大開刀了，可能一段時間無法工作，只得利用這方法事先籌措生活費。她一邊說一邊流淚，也懇求我的原諒(還一度雙膝跪地，被我連忙拉起)，當下聽了有點震撼，也心中不忍，但再怎樣，健保和私人醫療保險都是所有被保險人的公共財，不能如此被濫用，隔天還是請他們出院，並請社福部門介入處理。
事後從社工那裏了解她們家的處境，媽媽並未和先生離婚，但先生長年不顧家， 家裡生計均由媽媽一人扛起，也算是另類的單親家庭。
台灣社會算是富裕，但還是會上演「朱門酒肉臭， 外有凍死骨」的戲碼，落入貧窮線底下的家庭，連最基本的生活需求都要靠這種手段才能達成，著實令人心噓不已！ 雖然事隔多年， 腦海還是會浮現這對母子的身影，聯醫目前推行的「全人醫療」，就是秉持這個理念，醫生和病人之間，除了醫病關係外，也還有一些價值在其中。

你知道嗎？【歇斯底里跟子宮的關係】

「歇斯底里」是指由於未知恐懼等原因而造成的情緒失控，中國傳統習俗認為是中邪或是鬼上身。

歇斯迪里（hysteria）這個洋名，其實源自於古希臘文的「子宮」（hystera），古希臘人認為歇斯底里是未婚女性的子宮在體內遊走導致，對古希臘人來說，子宮是一個獨立的個體，會在女性體內遊走「阻礙呼吸與產生疾病」[1]。

就算到了16、17世紀，歇斯底里還是被認為是子宮在體內的遊走，而原因是因為性慾沒辦法被滿足，而在社會宗教氛圍濃厚的情況下，自慰並不是一個選項，所以走入婚姻才是根治這個病的解方。

「子宮遊走」這個錯誤概念直到19世紀末才被導正，認為歇斯底里是個人無法因應重大壓力而引發的症狀，女性、男性都會發生[2]。目前在醫學的命名上，已經不使用歇斯底里這個詞，而以較為相關的「轉化症」和「解離症」來討論。
